Coaches and athletes request scratches using this, or a similar form. After the scratch is recorded properly, the admin official or computer operator mark and file the request form with other meet paperwork. Admin may wish to record other types of computer changes on the Computer Change Form.
This is a generic Scratch Request Form. The Microsoft Word format makes it easy to modify if desired. Not all meets use this form. Some prefer this form in duplicate so that the swim meet admin area keeps the original and the coach or athlete requesting the scratch keep the copy.
