Slips

Most swim meets use paper slips or logs to collect and report disqualifications (DQs). USA Swimming has a DQ slip pdf available on their website in the Officials Documents section. One can also purchase two part USA Swimming Disqualification Report Slips in their online swim shop.

Logs

A DQ log offers a less expensive alternative. It can also save time and streamline the recording process for the referee of high-DQ swim meets.

Electronic DQ Form

The electronic DQ form offers a free, non-paper alternative for reporting swim meet disqualifications.

Admin Disqualification Record

At a swim meet, an admin official or computer operator sometimes keeps a running record of disqualifications (DQs). This is a generic Disqualification Record. The Word file is easy to modify, if desired. The PDF file is quick to print and use. Not all meets use this form.

If officials use radios at the swim meet, the DQ Record can be used to record calls heard and track whether or not the call was accepted. Admin personnel can then review the log before finalizing event results to ensure all potential DQ’s have been appropriately closed. If using the log in this way, add titled columns as needed, especially if multiple administrative personnel will be using the same log.

Governance Tip  Governing bodies have different ways of recording disqualifications (DQ’s.)  If needed, contact the meet referee, or applicable meet governing body, for more information on how to record DQ’s.
